校園健康管理確診、及與確診密切接觸者通報處理

本所有關防疫措施說明如下
1.確診通報:本所教職員工生PCR如為陽性確診請填報本校系統: https://my.ntu.edu.tw/ntuwdc/ConfirmedReport.aspx

2.確診通報者請配合本所防疫聯絡人進行疫調。
3.如與確診密切接觸者通報,請協助填寫附檔EXCEL表格後,E-MAIL本所防疫聯絡人。

有關校方防疫規定如下:
一、確診或快篩陽性者
   1.       請通知您所屬單位/系所的防疫聯絡人,同時到本校防疫網站「確診者通報」系統(https://my.ntu.edu.tw/ntuwdc/ConfirmedReport.aspx做通報
   2.       進行7天居家隔離(可1人1室),期滿無需採檢可直接解隔,進行後續7天自主健康管理。
   3.       7天自主健康管理期間,應遵守衛生福利部公告之「自主健康管理對象應遵守及注意事項」,請做好個人防護,可以上課,亦可以進入辦公室、研究室、及宿舍中。

二、返國檢疫者
   1.       請通知您所屬單位/系所的防疫聯絡人,同時到本校防疫網站「居家檢疫通報」系統(https://my.ntu.edu.tw/ntuwdc/reporting.aspx)做通報
   2.       進行3天居家檢疫,以及後續4天自主防疫。
   3.       3天居家檢疫為 1人1戶,或入住防疫旅館,不得外出。
   4.       4天自主防疫可返家1人1室,以單獨房間含獨立衛浴設備為原則,但不可入住一般旅宿,亦不能進入本校宿舍。

三、確診者之密切接觸者
   1.       確診者的同住家人/室友
    (1)   請通知您所屬單位/系所的防疫聯絡人,同時到本校防疫網站「校園健康管理通報」系統(https://my.ntu.edu.tw/ntuwdc/internalReport1.aspx)做通報
    (2)   如已完成3劑 COVID-19 疫苗接種,可進行7天自主防疫,或3天居家隔離及4天自主防疫。
    (3)   如未完成3劑 COVID-19 疫苗接種,則必須進行3天居家隔離及4天自主防疫。
    (4)   3天居家隔離期間以符合1人1室(單獨房間含衛浴)為原則,不得外出。
   2.       確診者的自主應變對象(同職場、課堂、活動曾於2公尺範圍內接觸)
    (1)   與確診者在確診前2日內,有任何一方摘下口罩共同活動15分鐘以上之教職員工生,由本校提供1劑快篩試劑,快篩陰性無症狀者可繼續上班上課(無3日防疫假),如有症狀應儘速就醫。
    (2)   請通知您所屬單位/系所的防疫聯絡人。相關資料由防疫聯絡人填寫於確診者之疫調表中,不需上本校防疫網站做通報。

四、自主防疫者
自主防疫期間,教職員工生以不到校為原則;如有到校辦公辦事之需求者,可以到校(不需經直屬主管同意),但仍不能參加課程或進入宿舍,同時應符合下列規範:
   1.       個人須無症狀。
   2.       須有2日內家用抗原快篩陰性結果(例如 9/20 到校,需為 9/19 或 9/20 執行快篩結果陰性)。
   3.       到校期間全程佩戴口罩,維持社交距離,有飲食需求時可暫免佩戴口罩,並於用畢立即佩戴口罩。
   4.       禁止餐廳內用餐、聚餐、聚會、出入人潮擁擠場所或與不特定對象接觸。
   5.       延後非急迫性需求之醫療或檢查。


五、本校公費快篩試劑發放,依下列原則辦理:
   1.       校內確診者之校內密切接觸者(同住親友、同寢室友):每人發放3劑。
   2.       校內確診者之校內自主應變對象,且與確診者在確診前2日內,有任何一方摘下口罩共同活動15分鐘以上者:每人發放1劑。
   3. 領用方式:
    (1)   學生於宿舍內與校內確診者接觸(包含同寢室友、以及宿舍內共同活動15分鐘以上之校內自主應變對象):請逕向宿舍輔導員領取快篩。
    (2)   其他校內確診者之校內自主應變對象:
      A. 各行政單位之教職員工:請逕向所屬一級行政單位領取快篩。
         Faculty and staff of the NTU administrative units: please go to the top-level administrative units to pick up a rapid test kit.
      B. 各學院(系所)之教職員工生:請逕向所屬學院(系所)領取快篩。已從宿舍輔導員處領過快篩者,請勿重複領取。
    (3)   以上領用人員名單,應與保健中心確診者疫調表上的接觸者名單相符,且領用時須核對證件。


六、關於疫苗假及防疫假
   1.       申請方式:請至【myntu/學生專區/學生生活資訊/學生請假(學生)/學生請假系統】提出申請。
   2.       申請疫苗假:
    (1)   說明:學生於接種疫苗後若覺不適者,可申請疫苗假。若超過可申請疫苗假期限後,仍感不適就醫或休養期間,可依本校學生請假規則申請病假。
    (2)   請假天數:最多3日(含接種疫苗當日)。
    (3)   檢附證明文件:COVID-19 疫苗接種紀錄卡。
    (4)   疫苗假不列入出缺席紀錄。

   3. 申請防疫假:
    (1)   說明:因「居家照護」、「居家檢疫」、「居家隔離」、「自主防疫」、或學校相關規範等必要防疫措施等原因而無法上課,得申請防疫假。
    (2)   請假天數:依中央流行疫情指揮中心「具感染風險民眾追蹤管理機制」、或本校校級防疫小組規定。
    (3)   檢附證明文件:政府單位之相關正式通知、證明文件、或學校通知。
    (4)   防疫假不列入出缺席紀錄。

Dear NTU colleagues and students,

We would like to remind you of the following epidemic prevention measures before the new semester begins.


A.     Individuals diagnosed or tested positive for COVID-19
   1.       Please notify the epidemic prevention contact person of your unit/department and report to the Reporting confirmed cases system (https://my.ntu.edu.tw/ntuwdc/ConfirmedReport.aspx) of the NTU epidemic prevention website.
   2.       Please take 7-day home isolation (1 person per room). After the expiration of the period, you can release from home isolation without taking a rapid test. Please maintain self-health management for the next 7 days.
   3.       During the 7-day self-health management period, please abide by the Self-Health Management Compliance Items and Notice announced by the Ministry of Health and Welfare. Individuals can attend classes or enter an office, research room, and dormitory while maintaining self-health management.

B.     Individuals taking quarantine after returning to Taiwan
   1.       Please notify the epidemic prevention contact person of your unit/department and report to the "Reporting system for personnel under quarantine" system (https://my.ntu.edu.tw/ntuwdc/reportingE.aspx) of the NTU epidemic prevention website.
   2.       Please take 3-day home quarantine and take self-initiated preventive measures for the following four days.
   3.       Please take 3-day home quarantine following the principle of 1 person per household or stay at a quarantine hotel. Please dont go outdoors during home quarantine.
   4.       4-day self-initiated prevention can be carried out at home following the principle of 1 person per room with independent bathroom facilities. Please do not stay in hostels or hotels or enter dormitories on campus.

C.     Close contacts of confirmed cases
   1.       Family members or roommates of confirmed cases
    (1)   Please notify the epidemic prevention contact person of your unit/department and report to the "Reporting system for health management on campus" system (https://my.ntu.edu.tw/ntuwdc/internalReport1.aspx) of the NTU epidemic prevention website.
    (2)   If you have completed 3 doses of COVID-19 vaccination, you can take 7-day self-initiated prevention or 3-day home isolation followed by 4-day self-initiated prevention.
    (3)   If you have not yet completed 3 doses of COVID-19 vaccination, you should take 3-day home isolation followed by 4-day self-initiated prevention.
    (4)   Please take 3-day home isolation following the principle of 1 person per room with independent bathroom facilities. Please dont go outdoors during home isolation.

   2.       Individuals taking self-initiated responsive measures exposed to confirmed cases (having contact with confirmed cases within 2 meters at the same workplace, classroom, or event).
    (1)   Individuals exposed to confirmed cases for more than 15 minutes with either party having no mask on 2 days before testing positive will be provided with 1 rapid test kit. Individuals tested negative and with no symptoms can come back to the campus (with no 3-day epidemic prevention leave). Please seek medical attention as soon as possible if symptoms occur.
    (2)   Please notify the epidemic prevention contact person of your unit/department. The relevant information should be filled in by the epidemic prevention contact person in the epidemic report form of the confirmed person. There is no need to report it on the NTU epidemic prevention website.

D.     Individuals taking self-initiated prevention
During the self-initiated prevention period, faculty, staff, and students are advised not to come to the campus. However, one can still come to the campus (no need to apply for the consent of your immediate supervisor) without attending classes or enter dormitories. The following regulations should also be complied with.
   1.       Only come to the campus with no COVID-19 symptoms.
   2.       Only come to the campus after testing negative on a rapid test within two days before returning to the campus (For instance, if you intend to come to the campus on 9/20, you need to be tested negative on a rapid test on either 9/19 or 9/20).
   3.       Wear a mask at all times on campus and maintain social distancing. Masks can be taken off when dining and should be put back on immediately after meals.
   4.       Do not dine at school restaurants, attend gatherings or lunch/dinner parties, go to crowded venues, or have contact with nonspecific persons.
   5.       Non-urgent medical services or examinations should be postponed.

E.     Free rapid test kits are distributed on campus according to the following principles:
   1.       NTU community members that are close contacts of confirmed cases on campus (family members or roommates): 3 test kits per person
   2.       NTU community members that are taking self-initiated responsive measures of confirmed cases on campus, and being together with confirmed cases for more than 15 minutes with either party having no mask on two days before testing positive or confirmed with COVID-19: 1 test kit per person
   3.       The distribution measures:
    (1)   Students in the dormitory having contact with confirmed cases on campus (including roommates in the same dormitory room and individuals taking self-initiated responsive measures who have been in the dormitory with confirmed cases for more than 15 minutes): please contact the dormitory counselor to pick up a rapid test kit.
    (2)   Other individuals having contact with confirmed cases on campus and taking self-initiated responsive measures:
      a. Faculty and staff of the NTU administrative units: please go to the top-level administrative units to pick up a rapid test kit.
      b. Faculty, staff and students of the NTU colleges/departments: Please go to your affiliated college/department to pick up a rapid test kit. If you have already received a rapid test kit from your dormitory counselor, please do not pick it up again.
    (3)   The recipients listed above should be consistent with the list of contacts on the epidemic report form of the confirmed person, and the ID of the recipient must be checked when receiving rapid test kits.

F.      About vaccination leave and epidemic prevention leave
   1.       Application method: Please go to myNTU and access the Student leave system.
   2.       Appling for vaccination leave:
    (1)   Students who feel unwell after being vaccinated can apply for vaccination leave. If you still feel unwell after the period for which you can apply for vaccination leave, you can apply for sick leave following the NTU student leave regulations.
    (2)   Days of leave: Up to 3 days (including the day of vaccination).
    (3)   Supporting document: COVID-19 Vaccination Record Card.
    (4)   Vaccination leave will not be included in the absence record.
   3.       Appling for epidemic prevention leave:
    (1)   Students who are unable to attend classes due to necessary epidemic prevention measures such as "home care", "home quarantine", "home isolation", "self-initiated prevention", or other epidemic prevention related regulations may apply for epidemic prevention leave.
    (2)   Days of leave: Determined based on Tracking and Management Mechanism for People under Infection Risk announced by the Central Epidemic Command Center or the regulations announced by the NTU epidemic prevention team.
    (3)   Supporting documents: Relevant official notices or supporting documents issued by the government or notices from the University.
    (4)   Epidemic prevention leave will not be included in the absence record.
